You MUST have a Pre-Op History and Physical completed by your primary care physician and in most cases lab work completed, at least two weeks prior to surgery, no earlier than 30 days prior. Your primary care physician may also require you to have an EKG.  You may print the form below to take to your doctor if they do not complete or print their own forms. The completed History & Physical, lab results and EKG (if applicable) must be forwarded to our office NO LATER THAN 5 BUSINESS DAYS PRIOR TO YOUR SURGERY TO AVOID CANCELLATION.

If your insurance plan requires a referral, you will need an "Outpatient Medical/Surgical Center" Referral written by your primary care physician. This must be forwarded to our office PRIOR to your surgery in-order for us to bill your insurance on your behalf. Upon completion, you may either:
