New Patients: Please print and complete the "New Patient Registration Form" (in BLACK ink). Upon completion you may either:


  • Fax to Dr. Barry S. Tatar, M.D. LLC, at 410.799.3944
  • Email to scheduling@drtatar.com
  • Mail to 8178 Lark Brown Road, Suite 101, Elkridge, MD 21075
  • Bring it with you to your appointment

Please also include a copy of the front and back of your insurance card, if possible.
