If you have any questions on your individual insurance plan (what it covers, what your benefits are, deductibles, coinsurance, etc...) please contact your insurance plan directly. ·         If you have ANY form of MEDICAID/MEDICAL ASSISTANCE we can NOT      schedule you an appointment at this time, even if you have any other health insurance plan.

·         Your copay is due at the time of your appointment. We accept: Cash, Check, Visa,  MasterCard, American Express & Discover

·         If your plan requires a Referral, YOU must provide a copy PRIOR to your appointment or you will be rescheduled

·         If your plan has a deductible and/or co-insurance, you may be billed accordingly.

.         We will bill the above listed insurance companies on your behalf as a courtesy, however, it is ultimately your responsibility to make sure your account is paid in full and in a timely manner
